CARE DELIVERY MODEL

Our CARE DELIVERY MODEL has been designed to achieve the goal of taking care of our patients with an emphasis on being person-centered. We acknowledge that everyone’s needs are different and unique, and we strive to meet and exceed those needs. We personally develop a deep understanding of each patient’s needs, priorities, religious and cultural preferences, which enables us to provide the best care possible. We take the time to identify any barriers and challenges a person may be experiencing and we fill in those gaps by tailoring a treatment plan that is specific to the needs of the patients that we care for.

We strive to ensure a continuum of care for our patients to ensure that each person’s needs are met with dignity and we do this by taking the time to learn about each patient with our comprehensive assessments and treatment planning along with coordinating care with other providers to make certain the best course of action and care for the individuals we serve.
